Hospital discharge qualified social worker role

We have an exciting opportunity for 2 ARCH Social Workers to join our supportive team at Stepping Hill Hospital/and Pathway 1 and 2. In this role, you will work independently to facilitate patients’ transitions back into the community, emphasizing the “Home First” ethos.
The position involves discharge planning and multidisciplinary team (MDT) working from the hospital wards. You will also support the Transfer of Care hub when required and have the opportunity to support across our Discharge to Assess (D2A) bed-based provision. Additionally, we have a Reablement community team who work alongside our provider Service offering Care Act assessments and support planning within the Community.
We are looking for experienced hospital pathway social workers who have excellent skills in MDT work, discharge planning, carrying out Care Act assessments, and safeguarding work. We are a dynamic and fast-paced team who work collectively with the D2A to achieve the best outcomes for people. We are looking for confident and dynamic social workers who can work under pressure, have good problem-solving skills, good communication, and an excellent MDT approach. The work will include a combination of hospital discharge work, supporting the discharge from our pathway 2 bed bases or supporting pathway 1 in same day discharge processes where Care Act assessments are completed with people in their homes. The role will include a combination of duty, attending MDTs, and supporting colleagues to work collectively as a team. The social worker must have sound knowledge and experience of the Care Act legislation, Safeguarding duties, and hospital pathway processes to work within a person-centred ethos.
You must have:
1. A professional qualification in social work
2. Social Work England registration.
3. Enhanced DBS/Update service.
4. References required for the last 5 years of employment.
