The ethos of the team is a rehabilitative, person-led approach to recovery and self-management, ensuring we offer care close to home, avoiding unnecessary admissions to bed-based services and supporting timely discharge from hospital. The post holder will be part of our Core team and will work as an integral member of the Integrated Community Team to provide a service through assessing, collaborative goal planning, delivering, and evaluating treatment for adults over the age of 18 with a physical disability as follows:
1. Using recognised models of practice and evidence-based interventions and outcome measures to underpin all practice.
2. Assessing and treating own caseload of patients/clients, adopting a health coaching approach for all intervention.
3. Providing supervision and support to OT Assistants, Students, and Reablement colleagues as appropriate.
4. Providing occupational therapy advice to other disciplines and promoting the role of OT within the wider team.
5. Participating in departmental research/project work when opportunities arise.
6. Carrying out evidence-based assessment, intervention, and evaluation of occupational performance needs of patients with physical disabilities. You will assess, provide advice, provide appropriate standard or specialist equipment, refer on for minor adaptations, and liaise with the borough councils and housing associations in relation to major adaptations.
7. Be competent in risk assessments and managing risk autonomously within the home environment.
8. To advise other professionals e.g., district nurses, carers.
9. Liaise with partner agencies to ensure patients access to environmental adaptations via the Disabled Facilities Grant process.
10. To manage a diverse caseload of patients with complex needs.
11. Understand the nature, aetiology, and expected progression of a wide variety of diseases/disabilities and to plan appropriate interventions.
12. Understand the psychological and sociological impact of disease/disability on patients and carers.
13. Provide training, advice on lifestyle changes, and adaptations to the physical, psychological, and social environment to meet the occupational performance needs of the patient.
14. To plan and implement individual interventions by setting goals in collaboration with the patient.
15. Attend appropriate specialist courses and cascade training to colleagues.
16. To monitor and support supervisees e.g., Occupational Therapy Assistants, students.
