The post holder will have a specific focus on improving all aspects of discharge, supporting the wards/departments through strong clinical leadership, service improvement and development, and improving the patient’s experience of safe, timely discharge.
Main duties of the job
* Part of a multi-disciplinary team within the Trust Discharge Team providing discharge planning support and case management for complex patients.
* Ensure patients and their families/carers are consulted and encouraged to be part of the discharge planning process.
* Oversee the case management and co-ordination of discharge plans for patients requiring complex support.
* Ensure patients on a complex discharge plan are reviewed against their actual plan and take appropriate action when there is any delay.
* Ensure smooth and timely communication between the Trust and partner agencies.
* Support the Trust in managing the patient pathway and flow.
* Ensure that all areas within the Trust are aware of key issues pertaining to discharge planning.
* Act as a role model for all staff in the organisation.
* Ensure key performance indicators relating to discharge planning are met.
Person specification
QUALIFICATIONS / TRAINING
* Evidence of Continuing Professional Development in specialist field of practice.
* Relevant Degree in own clinical specialty.
* Registered professional with HCPC.
* Must take responsibility for personal development and ensuring continuous professional development is a priority.
* Will undertake all mandatory training for the role.
* Identify and support delivery of training requirements for own area.
* Member of Professional Body.
KNOWLEDGE, EXPERIENCE & EXPERTISE
* Significant knowledge and expertise in discharge and transfer of care planning and co-ordination.
* Experience in assessing acute medical and orthopaedic patients.
* Recent operational management experience and demonstrable clinical expertise.
* Ability to support performance against Trust targets, objectives, KPIs.
* Understanding of patient flow/strategic bed management/discharge pathways across the health economy.
* Experience in managing a ward or clinical department or working with inpatients and the community setting.
* Experience in staff or student supervision and appraisals.

* Evidence of excellent communication skills.

* Evidence of conflict management resolution.
* Evidence of working autonomously and within a team.
