We are seeking an experienced and highly motivated nurse with a passion for palliative and end-of-life care to join our hospital-based Palliative Care Team. * Have significant experience at a senior level in palliative and end-of-life care.
* Hold a degree and a palliative care qualification.
* Possess excellent communication and leadership skills.
* Are committed to continuing professional development and education.
* Are working towards or willing to complete physical assessment and independent prescribing within the first 1-2 years.

Main Duties of the Job
* Provide expert clinical support to patients with both malignant and non-malignant disease.
* Manage a caseload of patients with specialist palliative care needs.
* Contribute to education, research, and the implementation of national guidelines.
* Work collaboratively within a multi-disciplinary team to enhance patient care.
* Support a 7-day specialist palliative care visiting service within the hospital.

Job Responsibilities
* Act as a clinical expert in palliative care, providing specialist advice, guidance, and supervision to clinical staff, patients, and carers.
* Promote best practice in palliative and end-of-life care through expert consultation and case management.
* Support the development of individualised care plans, ensuring they align with patient preferences and national palliative care standards.
* Work autonomously, managing a caseload of patients with complex palliative care needs while adopting the key worker role, as agreed with the Palliative Care Team Leader.
