The Role
As part of the Scottish Government Discharge Without Delay agenda and Home First initiative we have an exciting opportunity to join our new multidisciplinary Home Assessment Team which aims to reduce delayed hospital discharges by assessing needs and providing a period of rehabilitation in the home setting.
The Home First pathway encourages all health and care professionals to ask the questions “why not home, why not now?”at every stage and has been evidenced nationally to result in significantly improved outcomes with reduced risk of deterioration in function through unnecessary prolonged hospital admission. An assertive and renewed approach to positive risk taking, identifying what a person has as opposed to what they need, supports people in maintaining their independence. Home Assessment Teams are currently being established across North Lanarkshire to support this process.
We are looking to recruit an enthusiastic Team Leader with HCPC registration and either an Occupational Therapy or Physiotherapy background to operationally lead the Allied Health Professionals in the North West Sector Team. University Hospital Monklands will be the base, however post holders will be hybrid working between their community base site, the acute sites and domiciliary visits. The post holder will work closely with a North East Sector Team Leader to ensure North Lanarkshire wide cover with subsequent cross-cover as required. There may also be a requirement to carry out some work from home. The post holder will also provide significant and valuable input into the acute, as duty team lead within the hospital.

What we'll need you to bring
1. Degree in AHP profession/Social Work, supplemented by further training and short specialist courses to post graduate, level or equivalent.
2. Registered Member of the Health & Care Professions Council (HCPC)/Scottish Social Services Council (SSSC)
3. Previous experience working in a community or patient/individual setting.
4. Specialist knowledge, experience and continuous professional development of relevant conditions, pathology, policies and procedures associated with the area of practice.
5. Comprehensive knowledge of clinical guidelines and standards within the speciality/area of practice
6. Previous experience of leading and managing a team, and evidence of supervisory, training and mentoring experience.
7. Breadth of practical, technical and clinical knowledge and skills to autonomously manage own and others caseload of people with complex needs.
8. Positive relationship behaviours including highly developed person centred communication, motivational and interpersonal skills.
9. Leadership, management, organisational and influencing skills to affect change within the team/service.
10. Information, management and technology (IM & T) skills
11. Effective time management skills. Full driving License Holder for Health staff.
12. A full, valid UK/EU/EEA licence.
It would be great if you also have
13. Previous deputising or project lead experience
14. Experience in representing the occupational therapy/physiotherapy service in local and national forums
15. Flexible working patterns to meet requirements of the post

Please note this is a fixed term post for 1 year. Where a post only has temporary funding and an existing member of NHS Lanarkshire staff wishes to apply, this will be treated as a secondment. The employee MUST already have written agreement from their line manager to be released on a secondment before applying for the post.
Location
This role will be based in within University Hospital Monklands however you will be hybrid working between community base sites, the acute sites and domiciliary visits.
