Job Description To assist the Community Care team carry out procedures in relation to a clients community care needs assessment, ensuring current policies and initiatives set by Local and Central Government are applied. Undertake home visits to contribute in the completion of needs assessments in relation to services being provided by Health and Social Care Department. The Moray West Community Care Team covers the locality areas of Elgin, Forres and Lossiemouth. There are opportunities to work within established Multi-disciplinary teams throughout the locality area. The team provides a supportive environment for practitioners to be part of during the current challenges facing the Health and Social Care sector. The duties of the post involve Regulated Work with protected adults, as specified in the Protection of Vulnerable Groups (Scotland) Act 2007. Successful applicants for such posts will be required to become a Protecting Vulnerable Groups (PVG) Scheme member in respect of Regulated Work with this group as appropriate, or if they are already a Scheme member, be subject to a PVG Scheme Record Update, before any formal offer of employment can be made by Moray Council. Responsibilities To assist with the completion of support plans, SDSQ, reviews and other related documents, and provide practical assistance to case-holders where required. To review less complex service packages as agreed by Team Manager in supporting the Community Care Team. Use of various IT systems. Participate in team meetings, attend supervision sessions and participate in ERDP Dealing with Challenging Relationships and Environment. To be aware of Health and Safety issues as per council procedures and policies. The Individual Experience /Training in Local Government. Knowledge of Local Authority policies in relation to Community Care. Educated to SVQ3, HNC or equivalent in relevant discipline with Higher grade or equivalent in English Knowledge and experience of Microsoft office packages, including Excel and Access Excellent verbal and written communication skills. To be able to work under pressure & prioritise own workload. Be able to exercise own initiative and make decisions. You will be expected to travel efficiently and effectively between various work locations within Moray to meet the operational requirements of the Service. Due to the rural nature of Moray this is normally undertaken by use of a car. Ability to deal confidently and sympathetically with people who may not agree with decisions and policies.
