Job summary
The post holder will:
Support the central patient safety team through administrative and clerical tasks to deliver the Patient Safety Incident Response Framework (PSIRF) across the Trust.
Provide administrative support and cover to the Bereavement Office as required.
Main duties of the job
General Responsibilities
1. Maintain patient confidentiality and follow Trust information governance practices
2. Complete mandatory and statutory training
3. Prioritise and manage workload without direct supervision
4. Support the Quality Department and Organisation by carrying out any other duties as qualified and able
5. Adhere to a smart dress code
6. Develop and promote effective interpersonal/working relationships with all staff within the hospital and external personnel
7. Minimise the Trust's environmental impact wherever possible, including recycling, switching off lights, computers, monitors and equipment when not in use
8. Undertake other duties commensurate with this grade of post in agreement with the relevant line manager

Job responsibilities
Main duties Patient Safety:
15. Support the delivery of the Patient Safety Incident Response Framework (PSIRF) and standards
16. Develop and maintain a centralised log of safety actions and quality improvement projects following safety incidents
17. Ensure the centralised log is regularly updated with progress on actions and actions are closed in a timely manner
18. Ensure test of effectiveness is completed prior to the closure of actions / projects
19. Identify and highlight duplicate actions and quality improvement projects
20. Identify and highlight any safety action trends and themes
21. Ensure evidence to support completion of safety actions and test of effectiveness is uploaded to the incident reporting system
22. Monitor timescales for incident response completion
23. Support the preparation of agendas, minutes, action logs and papers for patient safety meetings and learning responses
24. Monitoring and actioning emails to the patient safety team inbox
25. Undertake general administrative duties to support the Head of Patient Safety photocopying, scanning and uploading of documents
26. Support the preparation of letters to families about ongoing incident investigations / learning response
27. Managing the Patient Safety Team diary; making, changing and managing meeting and training invites
28. Develop good working relationships with internal and external stakeholders
29. Support the management of incidents using the incident reporting system (datix)
30. Develop audit tools and collect data to support the completion of patient safety reports
31. Support the development and delivery of patient safety training
32. Support the development of local policies and procedures
33. Support the cascade of safety alerts and monitoring of actions
Main duties Bereavement:
Be kind, caring, empathetic and compassionate towards bereaved relatives
34. Draw upon an understanding of spiritual, cultural and religious needs to provide a person-centred approach to guide bereaved families and can deal sensitively with their emotional distress
35. To keep clear accurate records relating to each death at the Trust, deceased patients property and making the relevant post death notifications ( to the GP).
36. Signpost relatives to appropriate bereavement services and resources
Arrange appointments for bereaved families with viewing of their loved ones
37. Facilitate the release of the deceased to a funeral director
38. Facilitate the release of property and valuables to relatives and manage unclaimed property
39. Co-ordinate proceedings in the event of an infant death including: Liaising directly with the Labour Ward, Birth Reflections Team, Bereavement Midwives or appropriate department to ensure that all paperwork relating to an infant death is correctly completed Arranging appropriate transport for babies/foetuses to and from the hospital at which the postmortem is to be carried out
40. Attempting to trace a persons next of kin
41. To support the Mortality Improvement Lead in the preparation and submission of evidence for annual assessments / key performance indicators
42. Be fully conversant with all relevant national and local Policies and Guidelines
43. Access regular supervision and support via their line manager to support them with the emotional aspects of the role
