We provide high quality care and support to our patients and are looking for a Paramedic to join our friendly and experienced team.
Main duties of the job
This role will ideally be suited to an experienced Paramedic, and hold a valid HCPC registration and either completed or working towards FCP (First Contact Practioner) with a proven background in delivering advanced level patient centered care within a primary care setting. Though we will consider applicants who are considering a move into primary care, who are enthusiastic and passionate about providing high quality care to our patients and becoming an integral part of our experienced multi-disciplinary team (MDT).
The successful applicant will need to demonstrate safe clinical decision making and expert care, including assessment and diagnostic skills, complete home visits on behalf of the GPs and manage minor ailment and vaccination clinics.
Main duties of the job
Work alongside the medical team, to provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making to establish a diagnosis and management plan.
To provide assessment, treatment and diagnosis at point of first contact by attending to patients in a variety of clinical or non-clinical settings according to patients needs.
To undertake home visits in accordance with the relevant protocols. To refer patients to an alternative care setting or treat and discharge as appropriate.
Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services.
Perform investigatory procedures as required. Undertake the collection of pathological specimens including intravenous blood samples, swabs, etc. Perform investigatory procedures needed by patients and those requested by GPs.
Communicate proactively and effectively with all colleagues across the multidisciplinary team, attending and contributing to meetings as required and accepting referrals and referring to specialist services where appropriate.
Assess, diagnose, plan, implement and evaluate treatment and or interventions and care for patients presenting with an undifferentiated diagnosis, and patients with complex needs within the clinical and domiciliary setting.

We have 5 GP partners,3 salaried, Pharmacist, a full team of practice nurses, HCA's, a Mental health nurse ,First Contact Physiotherapists.We also have an extensive practice support team consisting of receptionist, administrators, medical secretaries and our management team.
The practice is based in Lenham close to Maidstone with good road connections from M20 ,with a practice list size of approx 10.200 registered patients.
The practice is an accredited training practice for student GP's and Medical students.
We are part of a strong primary care network, delivering improved access, high achieving QOF and enhanced services, rated GOOD by the CQC.
A full driving license will be required for home visits.
Job responsibilities
Tobe responsible for the provision of paramedical services, working as part ofthe practices multidisciplinary team, delivering care within their scope ofpractice to the entitled patient population. The paramedic practitioner will berequired to work autonomously in the practice and in the community (homevisits), providing a range of services such as assessment, diagnosis,treatment, telephone triage and referring patients appropriately, as well assupporting the management team in the reviewing of clinical policy andprocedure.
Thefollowing are the core responsibilities of the paramedic practitioner. Theremay be on occasion a requirement to carry out other tasks; this will bedependent on factors such as workload and staffing levels. The paramedicpractitioner will work within their scope of clinical practice and:
* Identify,signpost or refer patients at risk of developing long-term conditions,preventing adverse effects on the patients health
* Provideroutine care to patients as required in accordance with clinically based evidence,NICE and the NSF
* Examine,assess and diagnose patients and provide clinical care/management as required
* Triagepatients and provide the necessary treatment during home visits
* Collectpathology specimens as required
* Maintainaccurate clinical records in line with extant legislation Ensurethat read codes are used effectively
* Ensurecontinuity of care, arranging follow-up consultations or reviews as necessary
* Reviewthe effectiveness of the treatment provided, making changes where necessary toimprove patient outcomes
* Chaperonepatients where necessary
* Prioritisehealth issues and intervene appropriately
* Actas the lead clinician when dealing with clinical emergencies
* Recognise,assess and refer patients presenting with mental health needs
* Supportpatients in the use of their prescribed medicines or over-the-counter medicines(within own scope of practice)
* Liaisewith external services/agencies to ensure that the patient is supportedappropriately (vulnerable patients, etc.)
* Ensurethat they adhere to the relevant patient group directives and local clinicalpathways at all times
* Supportthe clinical team with all safeguarding matters, in accordance with local andnational policies
* Understandpractice and local policies for substance abuse and addictive behaviour,referring patients appropriately
* Deliveropportunistic health promotion where appropriate

* Supportthe practice audit programme, undertaking audits when necessary

* Continuallyreview clinical practices, responding to national policies and initiativeswhere appropriate
* Participatein the review of significant and near-miss events, applying a structuredapproach, i.e. root cause analysis (RCA)
