An exciting opportunity has arisen for a Band 6 clinical pharmacist to join our Pharmacy Department.
This post is offered on a Band 6 progressing to Band 7 on successful completion of Postgraduate Diploma in Clinical Pharmacy and suitable performance.
Pay progression is locally agreed and includes a pay increase on diploma enrolment, a further increase on enrolment onto the second year, and finally progressing to band 7 on passing the diploma and suitable performance.
We have vacancies for enthusiastic and motivated pharmacists to complete our current team of band 6 rotational pharmacists and participate in the provision of the Trust's pharmacy service.

Main duties of the job
* The department operates a 7 day service to the medical care group and as part of this role you will be expected to participate in the rostered shift system which is currently 1 in 5. Participation in the late night working and pharmacy on call will also be required.
* The Trust comprises two modern high-performing state-of-the-art PFI hospitals, St Helens and Whiston, which have accredited teaching hospital status.
* The Pharmacy Department has over 80 staff operating across the two sites. We have established automated dispensing systems within both dispensaries, and there is an ambitious and active technology implementation program which includes automated labelling & checking.
* A new e-prescribing facility has recently been launched. Both the Department and the Trust have a long-standing reputation for being a friendly and rewarding place to work.
* A previous Investors in People assessment recognised our Pharmacy as an outstandingly good department for staff support and development.
Job responsibilities
KEY DUTIES
* Taking drug histories on admission and informing medical staff of any discrepancies with medication prescribed to ensure continuity of treatment.
* Review medication to ensure safe and effective use.
* Annotate patients' medicines kardex with details of any allergies and specific instructions relating to administration of medicines to ensure safe and effective therapy.
* Formulate individual pharmaceutical care plans and implement appropriate monitoring of therapy to ensure safe and effective therapy.
* Review and update care plans depending on response, results of investigations and adverse effects to ensure safe and effective therapy.
* Educate patients on their drug therapy to enable concordance.
* Liaise with nursing and medical staff on issues relating to patients' drug therapy to ensure safe and effective therapy.
* Checking patients' own drugs for suitability to reuse to ensure safe and effective therapy and achieve medication cost containment.
* Therapeutic drug monitoring for specific drugs with a narrow therapeutic window to ensure safe and effective therapy.
To provide professional and legal clinical supervision in the dispensary as allocated: Duties will include,
* Professional checking of inpatient requisitions, outpatient, Accident and Emergency, and To Take Out (TTO) prescriptions to ensure safe and effective therapy.
* Ensuring all prescriptions comply with the Medicines Act, and the Misuse of Drugs Act where appropriate.
* Final checking of prescriptions for accuracy prior to release including extemporaneously dispensed items.
* Labelling and dispensing of medication as required including extemporaneously dispensed items to ensure compliance with relevant legislation.
* Providing advice to patients on how to take their medicines and potential side effects to ensure safe and effective therapy.
* Advice to Doctors, nurses and other professionals as required.
* Receive and dispense controlled drugs in accordance with the Misuse of Drugs Act.
* Dispense extemporaneously on call when required to do so.
* Liaise with medical staff on the availability of medicinal products.
Person Specification
Qualifications
* Vocational master's degree in pharmacy or equivalent as deemed by the General Pharmaceutical Council.
* Registration with The General Pharmaceutical Council at the time of appointment.
* Willingness to undertake a Post Graduate Diploma/MSc in Clinical Pharmacy.
* Membership of the Royal Pharmaceutical Society.
* ECDL.
Knowledge and experience
* Sufficient number of months pre-registration training.
* Experience in hospital pharmacy.
* Good communication skills.
* Ability to work accurately and efficiently.
* Well-organised with good administration skills including record-keeping.
