The Pennine Care Community Rehabilitation Team is looking to recruit a motivated and enthusiastic Occupational Therapist to join our supportive multidisciplinary team and contribute to this exciting endeavour. The successful candidate will have significant experience working with people with psychosis across a range of services. The team supports service users to transition from inpatient rehab facilities to the community. The aim of the team is to reduce the length of inpatient stay for service users from both acute and rehabilitation wards, incorporating least restrictive practice, the NICE guidelines for complex psychosis and the Getting It Right First Time (GIRFT) national program. The team has a strong emphasis on the Recovery Model, working with service users to maximise their independence and autonomy to improve their quality of life. You will work with a team of experienced practitioners with a strong focus on psychological formulation driven, evidence based interventions. You will receive regular supervision and your personal development priorities will be shared and supported through our appraisal process. The team is a trust wide service covering all five boroughs of Pennine Care so it is essential that you have access to a car during working hours. We are currently based at Trust HQ in Ashton Under Lyne although this may be subject to change. Our core hours are Monday to Friday 9am-5pm. To embed the Occupational Therapy assessment and intervention pathway across the Community Rehabilitation Team, within the framework of Model of Human Occupation in collaboration with the wider MDT. To be responsible for a caseload of service users under the care of the Community Rehabilitation Team, supporting them in their recovery journey. To undertake specialist OT assessments in all areas of self-care, productivity and leisure to support discharge planning and identify support needs. To conduct OT assessments, developing outcome-based needs led care plans, facilitating the implementation of the agreed plan of care and evaluating its effectiveness utilising evidence-based outcome measures. To provide a range of highly skilled interventions that will be delivered on both a group and individual basis and share those skills with other team members. To develop treatment plans and intervention to be delivered by the team's social inclusion workers. To provide clinical supervision to the junior staff within the Community Rehabilitation Team. * Diploma or degree in Occupational Therapy and be registered with HCPC.
* Evidence of post qualification experience and continuing professional development to post graduate diploma level
* Substantial post registration experience working in a mental health service.
* Experience in working with people with severe and complex mental health needs
* Extensive knowledge / experience in social inclusion and recovery model of care.
* Sound understanding of current local and national policies in relation to mental health including the Mental Health Act, risk assessment and risk management processes.
* Good working knowledge of assessment, care planning, implementation and evaluation processes.
* Comprehensive knowledge of mental health conditions and how they impact on an individual's wellbeing and occupational functioning.
Desirable Criteria:
* Experience of working in a community setting.
* Experience of supervision
* Experience of managing a caseload
* Knowledge of MOHO and its assessment tools.
