Job summary PREVIOUS APPLICANTS NEED NOT APPLY Full time/Part time or Term time contract available The Children's Continuing Care Team is part of the Children's Outreach Nursing Service based at the Great Western Hospital. The team provides direct clinical care and support to named children and young people aged 0-18 years with complex health needs, including learning disabilities, life limiting and life-threatening conditions. Care shifts are provided in the child's home, school or other community setting. Shifts will cover both days and nights on a Rota basis, or for a term-time contract, day shifts during the school term. We are keen to recruit and train keen and enthusiastic support workers with good communication skills and a desire to make a difference to the life of children and their families in Swindon. Main duties of the job Successful applicants must be completely child and family focused with good communication skills. The children we provide care for and their families are central to the service, motivate the team, and we value their input into care planning and delivery. Candidates must recognise their own limitations, understand boundaries and be able to work as a lone worker in a community setting, supported by registered nurses following a period of competency based training to meet complex health needs. Candidates must be willing and able to work flexibly to meet the needs of the children and young people we care for.
