Job summary An exciting opportunity has arisen for a part time experienced Senior Therapy Assistant whohas knowledge, skills and experience in Intermediate Care, Rehabilitation, Rapid Responseand Admission Avoidance. We operate a 7-day service, currently covering the hours of 8 am to 8 pm, includingweekends; therefore, the ability to work shifts to cover these hours is a requirement of therole. Main duties of the job The Intermediate Tier prides itself on providing strengths-based assessments and interventions to the people of Bury, helping them regain or attain as much independence as possible within their own homes. Our patients truly are at the heart of everything we do, and we are looking for someone who is committed to being part of a healthcare team that delivers excellent care and support to our patients. To help us achieve this, you will be responsible for providing support work to aid the rehabilitation of patients. You will carry your own caseload, perform patient therapy assessments, develop, implement, and evaluate individualised holistic treatment programmes, and deliver a high standard of evidence-based care. Within all our Intermediate Tier services, you will work within and have support from a multidisciplinary team, follow appropriate pathways of care, and liaise with and refer to other agencies as appropriate. We seek someone hardworking and motivated with clinical, communication, organisational and time management skills. A flexible approach to day-to-day tasks is a must to meet the demands of the service. You will also need to travel across the service footprint, when necessary, in line with service needs, so access to transport is essential.
