Main duties of the job
* Participate in consultant ward rounds/MDTs to improve medicines optimisation by ensuring safe, cost-effective prescribing and compliance with Trust prescribing guidelines.
* Work alongside the Clinical Pharmacy Technicians to implement and deliver a 28-day one stop dispensing service, promote efficient use of patients' own medicines, and encourage self-administration of medicines.
* Support ward staff to maintain adequate pharmacy stock levels on allocated wards and regularly review and adjust levels based on prescribing trends.
* Involvement in regular audit work in defined areas of practice to help improve standards of care for patients of the Trust.

Person Specification
Experience
* Experience of working in a hospital setting (Community and/or Mental Health).
Qualifications
* Masters Degree in Pharmacy plus successful completion of pre-registration training year.
* Member of General Pharmaceutical Council (GPhC).
* Diploma/MSc in Clinical Pharmacy or equivalent.
