Job Purpose:
Demonstrate the ability to analyse complex clinical information, adhering to the national rules and conventions of ICD- 10 and OPCS classifications codes. To support Trust information, Service Line Reporting, Clinical Governance and to facilitate maximisation of the Trust income in line with PbR (Payment by Results)
Duties
* Using a range of complex and sensitive source documents both manual and electronic, to extract, analyse and translate clinical date for all inpatient and daycase finished consultant episodes, (FCEs) following the rules and conventions of ICD-10 and OPCS.
* This information often includes exposure to highly disturbing narrative of a patient’s diagnosis, prognosis and photographs.
* To act as a designated clinical coding lead and to advise on clinical coding by attending clinical and management and departmental meetings, to discuss complex coding rules/guidelines and to answer questions with regards to coding when necessary to Trust colleagues at all levels both clinical and non-clinical, verbally and in writing regarding complex coding rules, national standards and working practices.
* To be proactive in liaising with clinicians and other related healthcare professionals, to lead in ensuring consistency and accuracy of coding.
* To mentor less experienced clinical coders and clinical coding trainees, to assist the clinical coding coordinator in the supervision of band 3 staff and support band 4 colleagues, to assist in the training of more junior staff, providing guidance on allocation of work and to motivate and support trainees in supporting them in all relevant processes and methods used in the clinical coding department.
* To input coded data onto the Trust PAS using encoding software. To plan and work using own initiative to ensure accurate, complete and timely coding is achieved at all times, to meet departmental targets and deadlines and to inform the coding coordinator and clinical coding manager of any difficulty that impedes the achievement to these targets.
* To understand the impact of coded date on the business aspects of the Trust and so be able to provide information and advice to all health professionals and administrative staff of the nature and complexity of clinical coding and it’s purpose in areas such as patient care, clinical governance, clinical audit, finance, budgeting, data quality and research.
* Use initiative to follow up queries generated by the coding team, contacting relevant Trust clinicians, clinical coding manager and colleagues at other Trusts and where necessary escalating the query to HSCIC.
* Make informed decisions regarding any required changes to the coding of specific diagnosis and procedures whilst adhering to national coding standards. Assessing the impact changes could have to income and mortality indicators.
* Continue professional development and undertake appropriate and mandatory clinical coding training in ICD-10 and OPCS classifications. To undergo continual audit and monitoring of coded work as part of professional development, to take part in feedback sessions, to accept critical judgement of work and apply standards learnt as part of the clinical coding training process.
* To investigate and correct coded clinical data rejected or queried by information systems
* Participation in the internal audit process and in the ongoing development of the coding team, by communicating specialist knowledge in the form of in-house presentations and workshops.
* Effectively manage and prioritise own workload to ensure that all necessary work is completed within monthly targets and to ensure that quality and accuracy is maintained in accordance with national clinical coding standards and conventions to ensure a high quality of coded data.
* To support the clinical coding coordinator in managing the clinical coding office and it’s equipment and for taking responsibility for ordering stationary as supplies as and when required.
* Contribute in the awareness sessions for junior doctors and other healthcare professionals to develop an understanding of the role and importance of the clinical coding function which may involve presenting to large numbers of your peers.
* To be responsible for the maintenance and personal improvement of clinical coding knowledge, through a process of continuous professional development.
* To be aware of PbR (Payment by Results) understanding the relationships between ICD-10, OPCS and HRG coding systems and the impact of coding on income.
* To assist in the staff coding audit process on a monthly basis and to assist in the analysis and support in collating this information for presentation to the clinical coding manager.
Essential Qualifications, Skills and Experience
* National Clinical Coding Qualification (ACC) typically demonstrated with coding experience following the successful completion of the Clinical Coding Standards Course.
* Evidence of attendance at a Clinical Coding Standards Refresher Course within the last 3 years
