Job Role: Qualified Social Worker Adults Job Location: Liverpool City Council Salary: £43.78/hour Working period: Monday – Friday Hybrid Working Working Hours: 37 hours per week Assignment period: 3-6 Months Ongoing Main Responsibilities of Post: Qualified experienced Social Worker with vase knowledge of legislation, and good application of practical skills to support a very fast paced Adult Social Care Team. The Agency Social worker will be working within Adult Access Team, which is located at Cunard Building, providing assessment and reablement support, as well as assessing eligibility for ongoing support within the community. The duty to prevent, reduce and delay is alongside care and support provision and applies to all interventions. The team works with adults over the age of 18 and have a varied caseload including include safeguarding enquiries, hoarding concerns, substance misuse, older people, mental health, learning disability amongst others. Liquid Logic is the programme used and any worker will need to be able to utilise this proficiently. Agency worker will need to be able to complete thorough and detailed Reablement assessments and support plans, Care Act assessments, Mental Capacity Act assessments, carry out reviews, provision packages of care, day centre, supported living and respite placements etc. Agency worker will also need to participate in a Duty system to resolve cases that are not allocated. Agency worker will be working alongside a number of partner agencies in the Liverpool area and will have support from a Team Manager or Senior Practitioner to oversee cases and provide advice.
