We have a rewarding opportunity for three Community Support Workers (Positive Behaviour Support) to join our new service based in Wythenshawe supporting young people and families across Manchester. We need to expand the service as it's proving to be so successful. Working therapeutically within a person-centred PBS model you will support young people and families to stay together and thrive. Our ultimate goal is to improve the quality of life for all those we support. The service is based in Wythenshawe, Manchester however you will work across Manchester supporting several families. Starting salary is £23,412.00 per annum based on working 37.5 hours per week. As a specialist service there is a comprehensive package of training and support either internally within the Trust's Learning & Development Team or externally with our professional partners (which include social services and CAHMS).
Responsibilities:
* Travelling throughout the Manchester area, you will visit a variety of settings that meet the young person's needs.
* Provide a caring, safe, secure and accepting environment that meets the physical, health, social needs and rights of the young people.
* Assist with the assessments of need and formulation of plans which includes working in partnership with other agencies.
* Maximise educational opportunities and achievements for the young people looked after within the unit by assisting in identifying, establishing, and supporting educational provision and contributing to educational reviews.
* Undertake individual work that is designed to meet the young person's needs as identified within the care plan, enabling them to reach their full potential.
* Under close supervision, take responsibility for key working a case load of families.
