An excellent opportunity has arisen for a Lead Nurse to facilitate the effective multi-disciplinary discharge planning and processes across all Directorates within the acute Trust.
This position is based at Colchester Hospital but will require working across our Community Hospitals also.
You will be required to work on the weekend rota as part of your contracted hours.
The Transfer of Care Lead Nurse will be responsible for the daily operational management of the service, including the line management, supervision and support of allocated staff within the team.
You will take the overall lead for developing a framework for the delivery of staff supervision, training and development for The Transfer of Care Hub.
