Job overview
An opportunity has arisen for a Band 6 Community Practitioner to join the Children & Young Peoples Services in North Cumbria within our Learning Disability and Behaviour Support Service.
We are keen to recruit a permanent Band 6 Community Practitioner with experience in working with children & young people with learning disabilities and complex needs, experience of collaborative working with both partner agencies and the multi-disciplinary team, and the ability to undertake specialist assessment and deliver evidence-based therapeutic interventions.
Main duties of the job
* To be responsible for the Assessment and Management of casework of young people and their families who have substantial and complex needs.
* Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies, and the public.
* In collaboration with the young person and Carer, develop individualised Care Plans, ensuring regular reviews to monitor progress and adapt Care Plans as necessary.
* Deliver evidence-based Clinical and Psychologically based Interventions.
* Coordinate Treatment Planning & Interventions, Reviews, and Discharge Planning.
* Provide clinical supervision to staff as delegated by Clinical Lead and Manager.
* Provide compassionate care that is based on empathy, kindness, respect, and dignity.
* Incorporate a client-centred philosophy into nursing practice and client-centred collaborative care.
Person specification
Education and Qualification
* Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist.
* Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
* Proven track record of post-registration clinical experience relevant to the post.
* Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults), and Child Protection.
* Experience of working within a community team.
* Experience of working with Children and Young People.
Knowledge and Experience
* Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives.
* 12 to 24 month band 5 qualified nurse experience dependent upon service need.
* Understanding of difficult to manage/challenging behaviours.
Skills and Competencies
* Evidence of MDT and multi-agency working.
* Proven ability to carry out and provide comprehensive written assessments of individuals with learning disabilities and complex mental health needs.
* Risk assessment and risk management/safety planning skills.
* Demonstrate effective written and verbal communication skills.
* Ability to adapt within a changing environment.
* Ability to work under pressure.
* Ability to work autonomously and within a team.
You must have appropriate UK professional registration.
