Can you make a difference to our resident’s lives?
We have an opportunity for a Registered General Nurse or someone with a Nursing qualification to join our specialist housing team as a Wellbeing Advisor based at Cathedral Park in Hereford. As this is a brand-new facility, you will have the opportunity to have real input and get to know the facility before it opens to residents.
Delivering our wellbeing service, you will implement health screening and wellbeing information sessions, self-help groups and individual health assessments for retirement village residents.
Here at Platform we believe in making a difference to people's lives and if you want to make a genuine difference with the work you do, this might be the right role for you!
The role is 17.5 hours per week, working Mondays, Wednesdays and Fridays.
What could you be doing?
Providing a personal and responsive wellbeing service for retirement village residents including health assessments, personal health programmes, home visits and wellbeing surgeries.
Following up the outcomes and advice from GP referrals with residents, supporting them to understand and follow the medical advice.
Offering health and wellbeing information sessions for older people.
Working as part of the allocation panel to assess applicants for retirement village accommodation and services.
Collating information regarding the use of the service, and the impact and outcomes achieved.
Referring and liaising with other services on behalf of residents as needed, to achieve the most direct and timely response to health concerns.
Undertaking clinical supervision for other Health and Wellbeing Advisors so skills and knowledge can be maximised within the team.
Some things we need from you
RGN qualified with current Pin number or a Nursing qualification
Experience working in nursing or retirement living setting.
Friendly and empathetic approach, responsive and ability to remain solution focused.
Great interpersonal skills, with the ability to communicate with people from diverse backgrounds.
