Senior Community Physiotherapist - South Rural

An exciting opportunity has arisen within the South Rural Community Therapy Team. We are seeking an experienced physiotherapist with a passion for delivering effective, appropriate, and timely therapy support for patients in their own homes, ensuring that they avoid unnecessary admissions to hospital and providing goal-focused rehabilitation.
The South Rural Team covers a beautiful area in Suffolk that includes Hadleigh, East Bergholt, and Holbrook and will be based at Beacon House, Ipswich.
Please note a driving licence and access to a vehicle is an essential requirement of this role.
This is a full-time position, with hours/pattern of work to be negotiated. Part-time hours may be considered.
We are fortunate enough to be co-located with our nursing and social care colleagues and work collaboratively as part of the South Rural Integrated Neighbourhood Team. This is an opportunity to work within a knowledgeable and experienced team, working closely with the wider MDT and other community networks. We are looking for someone that is flexible, motivated, and willing to work within a changing environment. We will provide support through in-service training and professional forums, Mandatory training, 1:1 and personal development meetings, as well as clinical rolling skills training via the practice development facilitators.

The Therapy service has a high profile within the Trust operating a 7-day service and thrives on innovation and challenge. You must be able to:
* Demonstrate experience of working within a multi-disciplinary setting.
* Show excellent clinical skills, providing holistic patient assessments to a range of patients.
* Demonstrate positive leadership qualities within the Integrated Neighbourhood Team and the wider MDT.
* Provide supervision and support to staff and students.
* Promote seamless transitions between hospital and home for patients in the South Rural area.

Person Specification
Qualifications
* Diploma or Degree in Physiotherapy.
* Health Professions Council Registration.
* Documented evidence of Continuing Professional Development.
* Membership of an appropriate professional body.
* Membership of a special interest group relevant to the role.
Knowledge
* A high level of ability to reflect on and critically appraise own practice and performance.
* Current knowledge and skills of moving and handling dependent adults and older people.
* A knowledge of primary and community service.
* An in-depth knowledge and understanding of the NHS Plan and NSF's relating to own and associated services and the implications for service provision.
* A specialist knowledge and application of current best practice in the rehabilitation of adults with physical disabilities.
* Ability to direct others towards evidence-based practice.
Skills
* Car owner/driver/access to a motor vehicle (role will involve travel to other sites that cannot be easily accessed by public transport).
* Effective leadership skills.
* High level of presentation and training skills.
* Current professional student practice placement educator training and associated skills.
* High level of skill in developing and maintaining effective working relationships with relevant stakeholders.
* Evidence of skill relevant to the rehabilitation of adults with physical disabilities gained through theory and practical course and experience.
* Standard computer skills including use of Microsoft and email.
Experience
* Experience of the supervision of staff and students.
* Experience of participating in research and audit.
* Experience of working with adults with complex physical, sensory and cognitive disabilities.
* Experience of working with a range of primary and community services including Health, Social Care Services, and the independent Sector.
* Leadership experience in a Health/Social care setting.
* Proven post-registration experience within a multi-disciplinary service.
