Job summary We are looking to recruit a Band 5 Mental Health Practitioner (Mental Health Nurse, Social Worker or Occupational Therapist) to the Early Intervention Team. We are a team that work with clients experiencing their first episode of psychosis. We are a multidisciplinary team that covers the whole of Sheffield. Main duties of the job Clinical: o With colleagues, carry out comprehensive assessments of an individual's health and social care needs in a variety of settings and agree the most appropriate course of action. o To assess, evaluate and manage clinical risk. o To formulate a plan of care based on identified need. o Deliver evidence based interventions as appropriate. o Review effectiveness of care plan at regular intervals. o To work collaboratively with service users, carers and partner agencies. o To maintain computer held care records in an accurate and timely fashion. o Undertake all work in a culturally sensitive manner. o Act in accordance with the Care Programme Approach, Care Management System, Acute and Schedule Care Pathways as appropriate. o Develop effective working relationships with Primary Care colleagues. Supervision: o Regularly review caseload to promote efficacy and contribute to the overall management of Service. o Deliver supervision and facilitate PDR with more junior members of staff. o Participate in own PDR process, identifying training and development needs, and pursuing relevant actions to maintain and improve level of competence and effective contribution. Education and Training: o Act as mentor to learners, keep up to date with relevant course and curriculum developments, and maintain 'sign off' status. o Participate in training relevant to role.
