An exciting opportunity has arisen for a motivated and dynamic Nurse to join our Clinical Haematology team as part of our Red Cell service. The ideal candidate will possess extensive clinical expertise and be a registered non-medical prescriber.
Working in the inpatient, outpatient and day case setting, the successful candidate will assess patients, plan, deliver and monitor the care and management of patients with Red Cell disorders, particularly our Haemoglobinopathy patients. LUHFT hosts the Specialist Haemoglobinopathy Service for Cheshire and Merseyside jointly with Alder Hey Children's Hospital, and the successful applicant will be a core member of this team.
Key Responsibilities:
1. Assess patients and plan, implement and evaluate care using specialist clinical knowledge and skills.
2. Review patients and provide care in inpatient, outpatient and day ward settings.
3. Contribute to a multidisciplinary approach to patient management.
4. Provide education and support to patients, carers and colleagues.
5. Participate in audit, research and service development within the Haematology service.
6. Perform specialist assessment of patients' nursing needs, plan, implement and evaluate care delivery using advanced clinical knowledge and clinical skills.
7. Lead clinical care by managing a patient caseload providing expert assessment, planning and evaluation.
8. Co-ordinate and manage nurse-led clinics (within specialty).
9. Establish and maintain effective communication with patients and carers/relatives.
10. Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery.
11. Recognise changes in patients' condition that require the intervention of others and refer on as appropriate.
12. To support, assist and encourage the multi-disciplinary team in resolving patient problems, by the provision of new and innovative models of case management.
13. To lead the specialist nursing contribution to the service, in the delivery of high standards of patient care.
14. Act as a role model and expert nurse providing in-depth specialist clinical knowledge to colleagues through MDT meetings, for patients and carers/relatives.
15. Undertake clinical supervision of nursing colleagues on an individual or group basis.
16. Provide leadership to the team of nurse specialists and work with the service lead to develop new roles/ways of working (High Impact Changes).
17. Ensure the effective and efficient use of physical and financial resources.
18. Lead consultations on and develop strategies for delivering effective care within a changing environment that improves the quality of care and health outcomes in the specialist area.
19. Responsible for monitoring health, safety and security of self, the team and others and promote best practice in the ward area.
20. Ensure systems are in place for risk assessment and minimization including promoting 'no blame' cultures in order that near misses and incidents are reported.
21. Take the lead in the investigation of incidents/complaints when required, including detailed reports/letters to complainants.
22. Demonstrate clinical leadership and challenge specialty and directorate boundaries to enhance and support the patient’s journey.
23. To identify and develop proposals and business cases as required for future service innovation.
24. Manage own and other resources effectively ensuring efficient service delivery.
25. Lead recruitment and selection of staff in line with Trust Policy.
Advocate:
1. Ensure that high standards of nursing care are given and maintained and act when standards are not being met.
2. Involve patients and carers in the planning and delivery of care.
3. Act as the patients' advocate and ambassador.
4. Lead the development of a patient-focused education and information-giving strategy within the specialty.
5. Recognise, prevent and diffuse potential areas of conflict and take a key role in the management of the outcome.
