Job summary An exciting opportunity has arisen for a D2A Co-ordinator to join our team at Somerset NHS Foundation Trust. The D2A Hub, based at County Hall in Taunton, provide comprehensive co-ordination and administration for the Discharge to assess / Intermediate care county wide discharge service. We have 1 permanent position available. Previous applicants need not apply. Main duties of the job You will join a team of 7 Coordinators who facilitate flow through the D2A service, ensuring, with support from clinical leads, maximum usage of capacity across the service. You will help coordinate and run the daily Discharge to assess pathway HUB, recording activity on electronic and paper systems to monitor individual patient level information, and provide data analysis and reports in various formats as required. You will also liaise with and coordinate D2A Keyworkers, providers, and referring organisations to coordinate the discharge of individual patients.
