Implement and evaluate individual specialised treatment programs for chronic disease patients Respiratory Care including Peak Flow Readings & Spirometry Diabetes Care Management Cardiovascular Disease and Hypertension Management Cytology & Women's Health including Pill Checks Immunisations for Adults and Children Would Care including Complex Dressings, Compression & Doppler Travel Assessments and Vaccinations Ensure accurate records of all treatments and consultations are recorded on the clinical system, using SNOMED codes and templates as appropriate Support patients in the use of their prescribed medication or over the counter medication (within own scope of practice), reviewing annually as required Support the practice to ensure targets are met for QOF, LES & DES Collaborate with other agencies and services to help patient's wellbeing Ordering and checking of stock and vaccinations Support and work together as required on any research initiatives with the Practice Visit housebound patients at home when required Contribute to the education and training of students and participate in staff inductions to include awareness of infection control Provide education by attendance at courses and study days as considered relevant to your professional on-going education Any other duties delegated that are suitable to the post
