An exciting opportunity has arisen for a Diabetes Inpatient Specialist Nurse (DISN) to join the multidisciplinary Diabetes Team based at Scarborough Hospital, but supporting cross site working. The post holder will be required to support the multidisciplinary diabetes team in the management of people with diabetes, both in the hospital outpatients department also.
There is much emphasis on patient empowerment for self-efficacy in the management of long-term conditions, and therefore the post holder will require excellent interpersonal skills. They will need to be able to motivate and lead others and act as an effective role model. The ability to be flexible, creative and adaptable are key personal attributes. Candidates will be expected to be able to demonstrate knowledge and awareness of the current clinical and professional issues affecting people with diabetes, incorporating high quality evidence in patient centred care.
Additionally, they will demonstrate competency in teaching (patients and healthcare professionals), supervision of team members and sharing good practice. Experience of working in multidisciplinary teams is essential, while a post registration qualification in diabetes would be ideal.
Main duties of the job
Based in the Diabetes Department at Scarborough hospital, the job holder will have a Trust wide responsibility to promote clinical excellence. Working jointly with the current lead In-Patient Diabetes Specialist Nurse, you will play an essential role assisting in the delivery and ongoing development of the in-patient diabetes service, including active participation in the roll out of best practice guidelines and protocols.
As an experienced nurse, you will support the co-ordination of in-patient diabetes care ensuring effective communication is maintained at all times, providing support to qualified and unqualified nurses within the ward/department. You will play a key role in the assessment of the needs of inpatients with diabetes across the Trust, identification of potential and actual problems, and implementation of an effective care plan. If required, the post holder will offer advice and support to Healthcare Professionals in the community to facilitate a smooth discharge.
To act as a role model, promoting quality evidence-based practice, supporting nursing and medical teams across the Trust and ensuring effective management of the in-patient diabetes service in the absence of the lead in-patient specialist nurse.
The post holder will be expected to support the lead in-patient diabetes specialist nurse in the undertaking of high quality audit and research designed to evaluate and improve the service.
