The successfully appointed Nurse Assessor will work under the guidance and support of the Band 7 Nurse Assessor Lead(s) to assess individuals for NHS Continuing Healthcare, in line with the National Framework for NHS Continuing Healthcare and NHS Funded Nursing care. This role may involve a combination of First Time CHC or FNC assessments, and CHC and FNC Reviews.
We are looking for a dynamic Registered Mental Health Nurse who possesses a good level of knowledge and understanding of the CHC process and the National Framework for Continuing Healthcare, who has experience of working within community Mental Health Services and has a good knowledge of mental health within older adult age groups, and who is passionate about providing comprehensive and well-written documentation to clearly demonstrate the needs of our older adult service users.
The successful applicant will be required to work in close collaboration with other professionals from Adult Care Services, the Integrated Care Board, Nursing Home staff, and other relevant care professionals; providing advice and expertise to service users, relatives, and other involved professionals.
Please note that our office base is at 1 Bowlers Green, Kingsley Green, Harper Lane, Radlett, WD7 9HQ, however, the successful candidate will be required to travel across Hertfordshire, so will need to be a car driver with access to their own vehicle.
Main duties of the job
Main Duties and Responsibilities:
* Co-ordinate and lead Funded Nursing Care and/or Continuing Healthcare assessments or reviews for service users, liaising directly with service users/families and other key stakeholders and relevant health or social care professionals.
* To complete accurate and detailed documentation, and ability to rationalise and explain any clinical recommendations made.
* The Nurse Assessor may be required to complete Continuing Healthcare Checklists, CHC reviews, FNC First Time Assessments or FNC Reviews, depending on the requirement for that individual. This role may also involve reviewing the appropriateness of 1:1 care within care home environments and making recommendations to ensure that the level of care provided is appropriate to the person's health care needs.
* This role requires the Nurse Assessor to liaise effectively with the relevant health and social care agencies, as well as with other independent agencies, with service users and with families or representatives.
* To promote good practice throughout the Trust in relation to the CHC processes.
* Identify and promote good practice in services for people under HPFT services, in particular helping to advocate for those who may lack capacity.

Clinical Responsibility
The Nurse Assessor will need to lead and document Continuing Healthcare assessments and/or Funded Nursing Care assessments. This will include frequent requirement for intense periods of concentration - please note that assessments are often conducted in the service users' own home or in care home settings where there can be an increased level of noise and distraction.
The Nurse Assessor will be required to:
* Complete CHC or FNC assessments that are clear, concise and detail plans to safely manage any risks that are identified.
* Use a variety of clinical skills appropriate to the needs of the individual and the clinical setting in which they are seen.
* Demonstrate a high standard of record keeping and documentation with adherence to Trust policies.
* Work with colleagues to develop and demonstrate clinical expertise and high standards of mental health practice.
* Promote care that is person-centred, reflecting current best practice, and challenges practice that may be detrimental to older people, which may include the need to raise safeguarding concerns when appropriate.
* Demonstrate extensive understanding of the legal and ethical issues in providing mental care. In particular having a good knowledge of the Mental Health Act, the Mental Capacity Act, The Care Act, and the National Framework for Continuing Healthcare.

The successful applicant will:
* Have a high level of competency to receive and give clinical information verbally and in writing from/to those receiving care, their families/representatives, to professional colleagues from health and social care, and to outside agencies.
* Demonstrate a high level of interpersonal skills, self-awareness, and empathy in all communications. The assessments can sometimes involve discussing information that the service user or representative may find upsetting or distressing.
* Ensure adherence to all aspects of patient confidentiality, documentation, and record keeping according to Trust and national guidelines.
* Will have the ability to communicate clearly and sensitively in complex and emotional situations.
* Will have an ability to negotiate and problem-solve in contentious and/or antagonistic situations where agreement is required. When agreement cannot be reached, the successful applicant will be required to communicate clearly and rationally to justify recommendations they are making, whilst ensuring that others' views are taken into account.
* Will have the ability to develop and maintain good working relationships with colleagues within HPFT and all agencies who they encounter to the benefit of the service users and the service.

Person Specification
KNOWLEDGE, TRAINING AND EXPERIENCE
* Registered Mental Health Nurse
* Knowledge and experience of Continuing Healthcare Process
* Substantial post-registration experience in a community and/or inpatient mental health setting working within Older People's services
AREAS OF EXPERIENCE AND KNOWLEDGE
* Experience of multidisciplinary team working
* A minimum of two years post-registration experience, one of which within older people's mental health services
* Up-to-date clinical knowledge.
* Knowledge of legislation related to mental health and social care (e.g. Mental Health Act, Mental Capacity Act, National Framework CHC)
