We are seeking a Band 8a Lead ACP to join our team here at the Great Western Hospital.
As an Advanced Clinical Practitioner (ACP), the role involves working within Urgent Treatment Centre alongside a team of Emergency Practitioners you will be central to the delivery of Urgent Care within the Great Western Hospitals NHS Foundation Trust. The post holder is required to deliver an advanced level of clinical practice working within local and nationally agreed protocols and guidelines.
The ACP will exercise independent judgement to assess, investigate, diagnose, plan, implement and evaluate the clinical care and management of Urgent Care patients. They will work alongside Urgent Treatment Centre (UTC) practitioners, doctors and Lead Consultant and support the clinical decision making within the unit. The post holder will promote the UTC both internally and externally whilst acting as an autonomous, accountable advanced practitioner.
Advanced Clinical Skills:
Undertake comprehensive clinical consultations with patients as an autonomous practitioner using own clinical judgement to assess, examine, diagnose, plan and evaluation care for patients presenting with an undifferentiated diagnosis with the Urgent care.
Perform advanced therapeutic interventions as required by the service using define local competencies.
Ability to request and interpret radiology requests within the guidelines and protocols for the clinical area
As an independent non-medical prescriber, the post holder will prescribe and review medications within the practitioner’s competency and scope of practice for therapeutic effectiveness, appropriate to patients needs and in accordance with evidence-based practice.
The postholder will lead and maintain a nursing culture which is open to change, new ideas, concepts and innovations.
Effectively lead when delegated and monitor the delivery of care to ensure the individual needs of the patients are met in a safe and timely manner.
Provide information to patients, relatives and carers regarding the treatment options assessments, investigations and follow up.
Lead when delegated and support the development, implementation and review of standards, policies, procedures and clinical guidelines.

Act as a role model promoting and demonstrating the standards of care expected and challenging poor practice.
Effectively lead and motivate the UTC practitioner and nursing team to ensure high standards of patient care is always achieved.
Effectively support and motivate staff in the provision of care and future service development.
Promote evidence based clinical practice and be responsible for continuous performance within Urgent Treatment Centre.
