Requirements:
* Conduct person-centred assessments and develop care plans to promote independence and meet identified needs within resource limits.
* Collaborate with individuals, carers, and other professionals, ensuring care services are person-centred and strength-based.
* Manage a caseload of adults with complex issues, safeguarding their safety and well-being while maintaining accurate records.
* Embed personalisation in practice, focusing on individual budgets, direct payments, and outcome-driven support plans
* Undertake safeguarding investigations, manage risks, and contribute to service development while adhering to council policies and professional standards.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
