We are looking for someone who supports services users across health and social care to join our Patient Safety Team across AWP in the delivery of the Patient Safety Incident Response framework. Main duties of the job The post-holder will work with service and professional leads, patient safety leads, service users and carers to develop robust and meaningful investigations to establish learning from events within the Patient Safety Incident Response Framework. The post-holder will be responsible for the collection and analysis of complex information from a number of sources, for producing high quality investigation reports, and for developing (with relevant professional and governance leads) an implementation plan. The post holder will be responsible for engaging with those affected by serious incidents, including bereaved families, to ensure they have an opportunity to participate in the investigation process. This will include speaking with patients and/or their families who wish to raise concerns or questions they have about the care and treatment delivered by the Trust and striving to answer those questions. The post-holder will support delivery units in patient safety improvement activities with the aim of improving patient safety and proactively reducing risks from unanticipated adverse events. They will participate as a member of the patient safety team to ensure compliance with regulatory and statutory patient safety requirements.
