An exciting opportunity has arisen for a Band 5 or Band 6 Community Mental Health Nurse to join Gosport and Fareham OPMH CMHT on NHSP.
Your main responsibilities will include care coordination of service users, duty work on rotation, mental health monitoring, and supervision and mentorship of junior staff and students.
The Havant and Waterlooville Community Mental Health Team operates from Aerodrome House and is well known for being approachable and open.
The Community Mental Health Team is focused on delivering a recovery service for clients with severe and enduring mental health issues. We support our staff in developing their clinical skills in attending postgraduate training such as physical health and nurse prescribing.
Due to the nature of this post, a car driver is essential with access to your own car to carry out home visits and attend patient meetings.
Main duties of the job
The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health Team to coordinate the care of a complex caseload of service users, supervising Band 5 Community Practitioners and Band 3 Community HCSWs where appropriate.
Working under the supervision of the Deputy Team Manager/Team Manager, the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery-focused and developed in collaboration with the service user and, where appropriate, their relative and carer.

Qualifications
* Registered Nurse, Mental Health Practitioner.
* Recognised Mentorship qualification.
* Able to attend and participate in all statutory & mandatory training events to enhance continued professional development.
* Willingness to complete further studies as necessary to meet service delivery requirements.
