The following are the core responsibilities of the Pharmacy Technician, but not limited and dependent upon workload and staffing levels. Support and guide our Reception Care Navigation Team with repeat prescriptions and medication queries Encourage and support patients to reconcile medications to facilitate bimonthly collections Act upon compliance issues, liaising with the appropriate clinician Identify patients for medication review. Support the MDTs to improve medication reviews, maintain an accurate recall system Effectively liaise with external services i.e. district nurses to ensure medications for housebound patients Input data to patient healthcare records as necessary Record and report adverse effects accurately and appropriately, informing the Clinical Team/registered GP immediately Use local protocols to ensure correct monitoring of high risk medications Use prescribing data to identify priorities to improve prescribing and patient care in conjunction with the Medicines Management Team Access clinical IT systems to document activities, run searches and audits. Promote medicines optimisation to improve patient outcomes and to reduce waste Develop relationships with the wider MSK team and promote networking Promote the use of ERD and EPS within the practice Reconciliation of discharge summaries and care home records Support clinical team with structured medication reviews Support with QOF and national and locally commissioned services
