Job summary An exciting opportunity to work in the Neurology Specialist Team has arisen. This role will involve autonomous, independent clinical practice. With a background of nursing, the post holder will work closely with Consultants Neurologists and clinical nursing staff within the NMSK Division and other divisions within the trust towards improving service for patients with neurological conditions. Main duties of the job The Cognitive Disorders Specialist Nurse Practitioner will work as an integral part of the Multidisciplinary team at the Cognitive Disorders Service Southmead Hospital. Their responsibilities will include: To work as an Advanced Nurse Practitioner in Cognitive Disorders Provision of nurse led clinics, face to face consultations with patients with a range of cognitive disorders, and reviewing medications, providing information and offering advice. Provision of a nurse led telephone advice line. Answering patient queries and writing correspondence pre first appointment, between appointments and following discharge to local services. Initial post diagnostic support, providing advice on how to access information on legal and financial matters and the role of statutory and voluntary services. Organisation of day case procedures for cognitive patients. To include coordination of diagnostic investigations and performing lumbar punctures. Liaison with local services and HCP to strengthen communication between tertiary and secondary dementia services and ensure smooth patient experience.
