Job summary
This exciting new role isfunded by the Grand Union Primary Care Network, as part of expanding GP teamsnationally. The successful candidate will be part of a large dynamic teamworking in a 35,000 patient GP practice based over 3 sites.
Main duties of the job
We are looking for individuals who will work with our team of GPs, nurses, pharmacists and admin staff to co-ordinate annual health checks for chronic disease and long term conditions. You will be working directly with patients to offer support at home and identify health care needs through a holistic assessment.

Job responsibilities
- Working with our team of GPs, nurses, pharmacists and adminstaff to co-ordinate annual health checks for chronic disease and long termconditions
- Work directly with patients to offer support at home, andidentify health care needs through a holistic assessment
- Create annual care plans for patients (PCSP personalised careand support plan)
- Alert our clinical team to any identified concerns includingsafeguarding issues
- Support patients to use decision making aids, provide writteninformation to support their health needs and signpost to other agencies thatcan help support them to maximise their health
- Assist with lifestyle changes on a personal or population level.This might include running in house programs, or co-ordinating with localagencies to encourage patients to improve their lifestyle to improve theirhealth and well being

Essential
1. GCSE grade A to C (4-9) in English and Maths

2. Health and Social Care NVQ/Management NVQ or equivalent
