Responsibilities:
* Evaluating and reassessing members based on feedback from interdisciplinary teams, caregivers and personal observation, and developing care plan elements accordingly.
* Assisting in the development of care plans, specific to communication, dysphagia, cognition and speech/language deficits, with frequent reassessments and care plan adjustments as members’ abilities and needs change.
* Recommending and implementing therapeutic interventions based on individual member needs and requests.
* Visiting in-home members and creating/executing comprehensive care plans based on immediate and ongoing needs.
* Educating family and caregivers on advanced care plans and in-home needs.
* Documenting daily clinical notes and screens for use in future interventions and cross-team activity.
* Observing meal/snack times for pervasive challenges surrounding food consistency, and reporting these observations to the interdisciplinary team along with recommendations for diet/liquid consistencies.
* Reviewing and participating in individual and group quality data analysis, as presented.
Due to the hands-on nature of this role, the ability to develop and foster positive, caring, productive relationships with elderly members and their families is essential, as is the desire to work in a unique environment with a unique, integrated approach to care services. The Speech Language Pathologist will be a highly visible and critical member of the Senior LIFE team, with significant influence and input with regards to member and center-wide initiatives. All applicants must be Pennsylvania-licensed speech-language pathologists with ASHA certification, with at least one year of experience in this role for an elderly community. Candidates must have significant clinical competency with geriatric populations, plus a valid state driver’s license.
