What we ask from you?
- Working in a multi-disciplinary team alongside Clinical and Operational Leadership to ensure high quality care delivery to service users.
- Provide specialist assessment and care planning in conjunction with service users and carers.
- Evaluate treatment plans and undertake discharge planning and promote self-care.
Responsibilities:
* Be responsible for an agreed caseload.
* Co-ordinate the work of the Team.
* Support development of skills and competencies in others.
Essential Qualifications and Training:
* Nursing Registration with the NMC.
* Degree in nursing (or equivalent).
* Mentorship/Assessor Qualification.
Essential Experience:
* Experience in MDT working.
* Community nursing experience.
* Evidence of providing clinical information and advice to others.
* Caseload management.
