The Post is in Poole, and will include managing the Inpatient Rehabilitation team at Alderney Hospital and providing an inreach service to Figbury Lodge.
The successful candidates will have proven ability in delivery of clinical interventions to adult/elderly patients in need of:
1. Urgent care to avoid admission to hospital, and/or
2. Rehabilitation and condition management for patients with long term conditions, and/or
3. Planning to support timely discharge of patients from an inpatient setting.

We are looking for an individual who will inspire and demonstrate leadership qualities through clear visible management. They will strive for continuous service improvement and will manage change through strategic thinking, use of negotiating skills, self-awareness and effective communication.
Clinically they will be continually responsible for the assessment of care needs, the development, implementation and evaluation of specialist programmes of care. They will ensure adequate and safe staffing cover is maintained for patient care, act at all times as an advocate for service users, carers and relatives.

This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.
