Develop trusting, supportive relationships with patients working with them in a variety of settings including on the phone, in GP practices, in the community and in people's homes. Help people to improve their health, wellbeing and social welfare by connecting them to a wide range of community services, projects and resources. Support people to increase their active involvement with their local communities, with the potential to strengthen personal and community resilience, and reduce health inequalities. Provide one-to-one and group health coaching support for people with one or more long-term health conditions, based on what matters to them - with the aim of empowering people to manage their own health and improve their health outcomes.
Deliver Health Connections projects within the community, such as Talking Cafes, Habits for Health groups and Live Well With Pain groups. Manage and prioritise a caseload of patients. Understand when it is appropriate or necessary to refer people back to, or on to, other health professionals or agencies. Support the promotion of PCN projects and communications, as well as the Health Connections website directory and activities.
Identify gaps in local service provision through speaking to patients and communities. Work with line manager, colleagues and community stakeholders to explore potential responses. This might include helping to set up new groups or initiating partnership projects. Attend and contribute to team, GP practice and PCN meetings and events as required.
Participate in regular supervision, reflective practice and continual learning.
