We are NOW recruiting 2 Reablement Leaders to join our compassionate team:
* An opportunity to provide maternity cover on a fixed term basis
* Plus, a second opportunity that has just been released to be part of our permanent full time staff

When joining our Community Reablement and Outreach team, you will work in providing an island-wide service to vulnerable adults in their own homes across the Island.

You will play a vital part in undertaking the following duties:
* Undertaking initial assessments
* Case coordinating people
* Ensuring our clients' reablement journey is appropriate to meet their needs
* Line managing a team of support workers
* Working in a fast-paced environment
* Collaborating with the Island’s hospital to complete initial assessments on ward rounds and identify goals to enable them to return home
* Gaining strong professional relationships within the health and social care sector
* Full UK driving licence and own vehicle is essential to meet the travel requirements for this role
* Business use car insurance is required as your personal vehicle will be used in connection with your employment when a pool car is unavailable
For you to bring out the best in this challenging but fulfilling role, having a flexible approach to roster patterns that includes weekends and bank holidays will be essential.
You will be at the forefront of service delivery, leading by example and ensuring high quality services for people using our provision and their families and/or carers.
