Role Responsibilities:
1. Manage and optimize medication therapy for patients, improving health outcomes and reducing adverse drug reactions
2. Conduct clinical reviews, contribute to multidisciplinary meetings, and work closely with other healthcare professionals
3. Provide guidance on medication prescribing and management in line with evidence-based practices
4. Undertake audits and participate in clinical governance initiatives
5. Offer training and support to junior pharmacy staff and healthcare teams
Requirements:
1. GPhC registered
2. Experience in clinical pharmacy, ideally within primary care
3. Strong interpersonal and communication skills
4. Ability to work independently and as part of a collaborative team
5. NHS experience desirable, but not essential
