Rough Sleeper Outreach Worker
Location: London N15 6HR
Salary: £26,250 - £34,650 per annum

Their specialist services include substance use, clinical, employment training and education, housing services, housing support, health, young people, and families’ services.
The Role
As an Outreach Practitioner, you will support people who are homeless or vulnerably housed to increase access to substance misuse support, sustain accommodation, and ultimately move into settled accommodation.
Your duties will include but are not limited to:
* Delivering satellite provisions at co-locations such as GPs, probation, hostels, etc.
* Providing harm reduction, brief interventions, and being responsible for care planning, risk assessing, and case management for service users allocated to you.
* Having previous voluntary or paid experience of working in a client-facing role and using active listening skills on a day-to-day basis.
Skills and Qualifications
* Experience within a similar role as well as experience within the sector is ideal.
* Proactive problem solver, empathetic, motivated to do this role, and passionate about delivering a high standard of care to some of the most vulnerable service users.
* Comfortable with lone working and at ease working in a variety of settings.
