Would you like to be part of our community Ageing Well Programme, including integrated service delivery to enable a Virtual Ward, enhanced health in care homes, and anticipatory care targeted at people living with frailty, multi-morbidity and/or complex needs to help them stay independent and healthy for as long as possible?
Virtual wards allow patients to receive the care they need at home, including care homes, safely and conveniently. They also provide an opportunity to narrow the gap between demand and capacity for secondary care beds, by providing an alternative to admission and/or early discharge.
Virtual wards are supported by an evidence base that demonstrates patient, system and public benefits. These include; increased patient choice and personalised care, allowing patients to be treated in home environment, reduced ED presentations and admissions, improved staff experience, with opportunities for flexible working and blended roles, with a mixture of in-person clinical care and provision of virtual clinical care.
Our service is delivered across Whitby, Scarborough and Ryedale Community, encompassing travel across these areas. The initial model delivery will be within Scarborough community, with a focus on admission avoidance and early supported discharge for people living with frailty in Scarborough.
Main duties of the job
We are looking for Registered Nursing Associates to work in our Community Team, delivering care in patients' own homes and Care Homes. There will be an excellent induction programme and clinical supervision support, alongside opportunity for professional development across team members. There will be opportunity to support development of technology enabled local systems.
The service will operate 7 days a week, for a minimum period of 08:00-20.00, incorporating a MDT with a wide clinical skill set, to ensure equitable care across the full geographical patch, and overnight nursing service. There will be a robust clinical triage process, accepting appropriate referrals from across multiple partner organisations and through agreed clinical diversionary pathways.
Applicants will have demonstrable post registration experience of delivering complex care in a community setting, effective clinical governance implementation, and evidence of continuing CPD relevant to this clinical area to Nursing Associate level. Applicants will have evidence of nursing practice skills, and be able to demonstrate the impact of this on practice change and clinical service development.

Minimum Requirements
* Registered Nursing Associate on the NMC register
* Understand requirements for NMC professional Revalidation
* IT skills for email, online staff record and learning system, clinical ordering systems, patient record systems.
* A good understanding of Community Nursing
* Demonstrates understanding of the role of the Nursing Associate in context of the nursing interdisciplinary team, and organisation.
* Understanding of the importance of following procedures and care plans
* Evidence of continued professional development (CPD)
* Experience of administering prescribed medicines safely and appropriately
* Understanding or application of relevant clinical practice/standards/audit within identified clinical area
* A broad understanding of relevant policy, legislation drivers and their application to clinical and service area
Experience
* Experience of working in teams under appropriate supervision as part of a multi-disciplinary team
* Experience of working with patients to promote their physical and mental wellbeing through health promotion.
* Demonstrable experience of working within the service area
* Experience of using SystmOne
Skills and Competencies
* Evidence of effective inter-personal communication skills
* Development of supportive relationships with patients, carers and other professionals
You must have appropriate UK professional registration.
