Job summary The Humbleyard Practice has an exciting opportunity for an experienced diabetes specialist nurse to work within their team, across their 3 practices. This position is offered on a part or full-time basis. In this role you will provide comprehensive care management to our diabetic patients, providing them with clinical care, guidance and support with their diabetes. You will work to policies and protocols and strive to improve standards of care to the practices patient population. Main duties of the job Liaise on regular basis with GPs, practice nurses and HCAs to assist in the development of the diabetes service. Contribute to the co-ordination of care between primary and secondary care diabetes services to ensure high quality of patient care and the continuity of services. Provide a pathway of diabetes nursing care for the patient, referring to secondary care, multidisciplinary teams and voluntary services as required. Maintain effective communication with patients, carers and professionals. Manage a complex caseload. Participate in formulating individualised patient treatment care programmes in conjunction with the patient, multidisciplinary team and initiating changes in management of care. Request and interpret diagnostic investigations and inform patients and GPs of results. Ensure patients, carers and health care professionals are given verbal and written information regarding planned care.
