Multiple Sclerosis Specialist Nurse

Job overview
We are looking for a highly motivated and enthusiastic Multiple Sclerosis (MS) nurse to lead our small community MS team in North Northamptonshire. Our team of specialist clinicians provides support to individuals aged 18 and over who have received a confirmed diagnosis of Multiple Sclerosis. We take a holistic approach to managing the symptoms of Multiple Sclerosis and offer support, education, and advice to both patients and their caregivers from diagnosis and throughout their MS journey.
The team provides links across a range of health care providers and other agencies to support and coordinate care for people with MS, liaising between primary, intermediate, community, and secondary care providers as well as the voluntary sector.
Main duties of the job
1. Work as an autonomous practitioner and effectively coordinate and deliver an expert nursing service within Multiple Sclerosis.
2. Manage patients/client caseloads.
3. Ensure that patients and their families are fully informed and supported by providing clinical and social support.
4. Work collaboratively with colleagues to provide a cohesive service for all patients.
5. Provide education to both patients and caregivers.
6. Collaborate with neurologist consultants and MS nurses in the wider team for delivery of all available treatment options.

Skills
* Proven communication skills with patients with long-term conditions.
Knowledge and experience
* NMC registered.
* Multiple Sclerosis specialist module at master's level.
* Car user and ability to drive to alternative properties.
* Leading and experience in audit and research to facilitate quality improvement.
* Community experience of long-term conditions.
* Non-medical prescriber.
* Knowledge and experience of DMT.
