An exciting opportunity to work within the Southwest transplant centre. Your role will be to assess, plan, implement and evaluate expert nursing care and provide specialist nursing support to potential live donors and recipients, as well as their carers. The post holder will have continuing responsibility for the co-ordination and organisation of individual treatment programmes throughout the living donor pathway and work to develop communication across divisional, regional, national, occasional international and organisational boundaries. This post is a key role in the multidisciplinary team. Undertake 1:6 on call for national offers.
The successful candidate should have demonstrable experience working within the area of renal medicine with proven evidence of good communication skills, organisational skills and experience of innovative practice.
The role will include a 1:6 (pro-rata) on call service for local and national kidney offers as a renal recipient transplant coordinator. The band 7 will be expected to deputise for the band 8 in her absence.
Our transplant unit supports the nurses in personal and professional development, research & publications and sponsorship to attend regional and national transplant conferences and training days.
Job responsibilities
1. Responsible for his/her own caseload of patients entering the live donor programme whilst working as part of the Multidisciplinary team.
2. To provide information and support to people coming forward as potential living kidney donors identifying their psychological, physiological, emotional, spiritual and cultural needs.
3. To conduct initial health screening of potential donors and facilitate recruitment and triage of referrals.
4. To provide education about the donation process including compatibility testing, investigations/screening, the surgical procedure, the legal approval and aftercare involved to enable the donor to make an informed choice.
5. To undertake social and physical assessment of the potential donor maintaining appropriate documentation on vitaldata and in the hospital records. To request relevant investigations following the living donor pathway, coordinating and facilitating the one stop live donor clinic to streamline the work up process.
6. To undertake venepuncture, cannulation, weight, blood pressure, urinalysis documenting all information on vitaldata.
7. Liaising with the H& I lab to test samples of DNA for tissue typing between donors and recipients. Discussing any complex matching with the wider MDT.
8. To triage referrals coming in from the different referral centres and continuing the assessment process. Allocating a local nephrologist to these potential donors and arranging for a desktop notes review.
9. To present potential donors to the multidisciplinary team for review, presenting their medical history, relevant results and details of the assessment. To follow up on any further information required and/or referral to other specialties.
10. Responsible for communicating highly sensitive and emotive condition related information to patients and relatives, e.g., unsuitability to donate. Informing potential donors of the outcomes of multidisciplinary discussions.
11. Coordinates timely referral to surgical colleagues for assessment, including facilitation of pre-operative assessments and anaesthetic reviews.
12. Refers to the human tissue authority to ensure that appropriate legal approval is gained for living donor transplants to take place. Follows guidance set by the human tissue authority.
13. Liaises with surgeons and theatre to ensure theatre dates are planned in a timely manner. Liaises with donors and recipients to book in these dates for the potential transplant to go ahead.
14. Facilitates donors and recipients matched within the kidney sharing scheme. Liaising with other transplant centres to facilitate transportation of blood samples and information relating to these potential donors/recipients. Agreeing potential dates for surgery and once established coordinating the donation on the planned surgical date.
15. Ensure that the transplant team, donor, recipient, ward and relevant consultants are fully informed of the planned transplant.
16. Coordination on the day of theatre to include liaising with ward and theatre staff, patients and their relatives. Support the retrieval and transplant process as required through the organisation of appropriate theatre times and personnel available. Liaison with surgical team and medical staff.
17. Completing relevant paperwork to notify NHSBT of donation and transplant and to keep them informed of any complications that arise post donation.
18. To arrange appropriate follow up of live donors post donation in the live donor follow up clinic ensuring that donors are seen no less than annually. Assessing these donors in clinic to review any potential risk factors following on from donation e.g., hypertension. Ensure discharge information is sent to referring centres following donor discharge to aid communication.
19. To facilitate the national reimbursement/loss of earnings policy for all living donors who either go on to donate or who are deemed medically unsuitable to proceed. Liaise with the commissioner within NHS England where required.
20. Increase the number of potential live donors by liaising and visiting regional centres to identify patients suitable for renal transplantation and provide education to ensure that all potential transplant recipients and families are offered the option of living donor transplantation.
