We are looking for a motivated individual who can work in our busy Rheumatology Department. The job is based at Medway Maritime Hospital and involves 4 clinics per week, in addition to in-reach referrals at Medway Maritime Hospital supported by Rheumatology consultants. The post holder will be expected to support our specialist nurses, answer calls received from GPs and patients within the locality, and with the support of consultants where required.

Responsibilities include:
1. Provide specialist Rheumatology care and management of patients in Rheumatology.
2. Assess, plan and deliver care plans for patients.
3. Perform clinical assessment, diagnoses and treatments with guidance from senior clinicians.
4. Collaborate with multidisciplinary teams to deliver coordinated care.
5. Respond to emergencies and participate in on-call duties as required.
6. Ensure all patient records are accurate and up to date.
