Social Worker - Adults

Up to £31 per hour
About the Role:
An exciting opportunity has arisen for a passionate and experienced Adult Social Worker to join our Learning Disability team at Bournemouth Christchurch and Poole Council. This is a care management role that requires a high level of autonomy and the ability to work effectively with people with complex needs.
* This is not a remote working role.
* The successful candidate will be required to carry out face-to-face visits in the local area and be office-based at least 2 out of 5 days.
* The remaining days can be worked from home.
Key Responsibilities:
* Carrying out Care Act assessments and capacity assessments
* Devising and implementing person-centered care and support plans
* Undertaking safeguarding enquiries
* Working closely with families and carers to ensure their needs are met
* Representing the Council at multi-agency meetings
* Providing advice and guidance to other professionals
Essential Requirements:
* Degree or Diploma in Social Work
* Current registration with Social Work England
* Experience of working with adults with learning disabilities
* Knowledge of the Care Act 2014 and Mental Capacity Act 2005
* Excellent communication and interpersonal skills
* Ability to work independently and as part of a team
