Wood End Health Centre has an exciting opportunity to join our forward-thinking, innovative 8000 patient practice. We are currently looking to appoint an experienced Practice Nurse to join our team.
The Practice is two miles from Coventry city centre and has mixed demographics including high levels of deprivation, offering the successful candidate an opportunity to really make a difference to our community. Our core ethos is to provide high quality, accessible Primary Care services for all our patients, designing care around their needs, whilst adapting to the ever-changing challenges and demands of Primary Care and the NHS.
Main duties of the job
We are looking for a UK registered nurse with a valid NMC PIN number and significant post experience.
1. Provide excellent patient care.
2. Must have good knowledge on Quality Outcomes Framework.
3. Must be able to relay clinical information and maintain accurate, comprehensive consultation records, EMIS web & Docman.
4. Must have PC skills; ideally will be able to use basic Word, Excel & email.
5. Able to work in a changing environment and implement change when applicable to provide high-quality health outcomes.

We are a friendly training practice, with three partners and two salaried GPs, and we also host medical students from Warwick Medical School.
We work closely and collaboratively with our PCN (Sowe Valley) who share in our values. We are a high QOF achieving practice and were rated as Good by CQC in 2022.
Job responsibilities
1. To provide nursing services to patients, including treatment, screening, advice, and minor therapeutic procedures.
2. To run special clinics for patients including family planning, child health surveillance, cryotherapy, vaccinations, cervical cytology, warfarin monitoring, and chronic disease management.
3. To undertake diagnostic tests including blood glucose monitoring, PEFR, and urine analysis.
4. To advise on and supervise patients' use of devices including MDIs, PEFR meters, urinary diagnostic stix, blood glucose monitoring, and nebulisers.
5. To take and dispatch safely laboratory specimens including blood, urine, swabs, and smears.
6. To provide ear syringing, treatment for cuts and bruises, dressings of wounds and leg ulcers, and removal of sutures.
7. To provide immunisations for all ages and diseases and to know the contra-indications and side effects of vaccinations.
8. To provide immunisations and health advice to patients travelling abroad.
9. To provide telephone advice to patients and occasional home visits where appropriate and within levels of competence.
10. To keep up to date with resuscitation techniques.
11. To ensure emergency drugs are kept up to date and to operate a system for checking expiry dates of drugs and vaccines.
12. To maintain the nursing and treatment rooms in good order.
Health promotion
1. To give lifestyle advice to patients including advice on smoking, alcohol consumption, diet.
2. To provide new registration health checks for all new patients registering with the practice.
3. To provide well-person advice such as breast and cervical screening and HRT.
4. To provide health advice to patients concerning risks of heart disease and cancer, including those with a personal and/or family history.
5. To provide other general and specific advice in line with national and local health priorities.
Management of Chronic Disease
This includes diabetes, asthma, hypertension, COPD, CVA/TIA, CHD, and CHD risk assessment and management.
1. Identifying significant abnormalities and monitoring and development of individual management plans; agreeing these as appropriate with the patient and other health professionals.
Participation in the annual influenza campaign (which may include paid overtime at evenings/mornings/weekends).
1. Recognising issues and gathering sufficient information to refer (e.g. drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems).
Administrative and professional responsibilities
1. Ensure accurate notes of all consultations and treatments are recorded in the patients' notes on the clinical computer system as appropriate.
2. Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.
3. Attend and participate in practice meetings as required; practice meetings are held quarterly, and the nurse on duty is expected to attend.
4. Assist in the formulation of practice philosophy, strategy, and policy and develop appropriate protocols.
