The Therapy Assistant Practitioner will provide general and specific care to adults, as specified below, and will be proficient and competent to work across therapy disciplines. The worker will be flexible and prepared to work as part of a dynamic team, willing to support the MDT and to adapt their approach to meet the needs of patients putting patients at the heart of what they do.
To be an integral member of the Chartridge Multidisciplinary Team (MDT), providing prompt delivery of therapy interventions and equipment to patients, in liaison with senior staff, which support improving a patient's function and discharge home.
To take responsibility for designated elements of the patient treatment protocols and delivery of such treatments in patients within their current location (Hub Bed Care Home, ward and occasionally patients' own homes).
To act as a technical resource in relation to the provision of mobility equipment.
To document, evaluate and liaise with qualified Physiotherapists and Occupational Therapists following interventions.
To be responsible for the completion and potential delegation of non-clinical tasks within the Chartridge MDT.

Person Specification
Education, Qualifications & Training
* Level 3 Health and Social Care Diploma or equivalent.
* Commit to undertaking Foundation Degree and any associated training once available.
Experience
* Significant experience working within a patient-facing environment with a reablement/rehabilitation focus.
* Previous experience working with patients with physical, cognitive or behavioural impairment.
Skills, Ability and Knowledge
* Collaborate with multi-professional service users, internal and external at all levels.
* Able to motivate self and be able to work with all levels of staff.
* Ability to work well in a team and independently.
* Able to accept and undertake responsibility for designated tasks.
* Clear, concise verbal and written communication to ensure liaison with multi-disciplinary team, patients and carers to ensure records and reports are accurate, legal and in a legible manner (in English).
* Demonstrate insight into the importance of maintaining a safe working environment.
* Ability to learn/ability to self-initiate learning (evidence within the last 2 years).
* Physical ability and endurance to work in the physical environment of the clinical area required by this post.
* Flexibility and adaptability to changing needs of the service.
* Able to work under pressure and to tight deadlines.
Special Circumstances - Ability to Drive
* Valid and current driving licence and car to be able to travel to locations throughout Buckinghamshire.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
£26,530 to £29,114 a year per annum pro rata.
