Job responsibilities Job Description Practice Nurse 24 hours per week (over 3 days) Responsible to: GP Partners Clinical Responsibilities: Chronic Disease Management: Provide comprehensive care and management for chronic conditions such as asthma, COPD, and diabetes, including regular monitoring, patient education, and collaboration with multidisciplinary healthcare teams to optimise patient outcomes. Conduct Peak Flow Readings and possibly Spirometry as required. Monitor and manage patients with hypertension, offering evidence-based interventions to reduce cardiovascular risks. INR monitoring. Travel Health: Deliver a holistic approach to travel health by advising patients on vaccinations, medications, safe sex practices, food hygiene, and sun protection to ensure safe and informed travel. Vaccinations: Administer childhood and adult vaccinations as per patient group directions, providing informed advice and ensuring adherence to immunisation protocols. Well Person Clinics & Health Promotion: well person clinics and health promotion initiatives, offering lifestyle assessments and advice on diet, smoking cessation, alcohol reduction, and physical activity to enhance overall health and well-being. Baby Clinic Care: Administer vaccinations Cervical Cytology: Conduct cervical cytology smear tests according to local and national guidelines, ensuring accurate documentation and patient care. Venepuncture: Carry out venepuncture procedures following local protocols to facilitate diagnostic testing and patient care. Wound Management: Provide a holistic approach to wound management. Contraception Services: Provide advice and administer contraception options, ensuring patient education and adherence to clinical guidelines. Suture Removal: Safely and efficiently remove sutures, ensuring adherence to clinical protocols and patient comfort. Safeguarding: Maintain awareness of statutory and local clinical protection procedures, including referral systems. Possess the ability to recognise signs and symptoms of child abuse, ensuring appropriate actions are taken to safeguard vulnerable patients. Health & Safety and Compliance: Sharps and Waste Disposal: Safely manage the disposal of sharps and hazardous materials in compliance with health and safety regulations, ensuring a clean and safe environment for both staff and patients. Infection Control: Strictly follow infection control protocols to prevent the spread of infections and maintain a safe healthcare environment, adhering to all national and local guidelines. Emergency Management: Recognise and manage anaphylaxis in accordance with current UK guidelines, providing prompt and effective treatment during emergencies. Perform cardiopulmonary resuscitation (CPR) as required, ensuring timely intervention in life-threatening situations. Health & Safety Protocols: Follow established procedures for handling and cleaning body fluid spills, maintaining a safe and sanitary environment in line with health and safety standards. Administrative Responsibilities: Informed Consent: Obtain and document informed consent (verbal or written) in accordance with legal and ethical standards. Stock and Equipment Management: Monitor and manage the maintenance of stock and equipment, including refrigeration and emergency equipment, ensuring that all resources are readily available and in good working order. Vaccine Management: Ensure the effective management of vaccine storage, including maintaining the cold chain and proper stock rotation, to uphold the integrity of all immunisation products. Confidentiality: Maintain the highest standards of confidentiality regarding all patient and practice information, ensuring compliance with privacy regulations and safeguarding sensitive data. Documentation: Ensure all consultations are documented in accordance with Nursing and Midwifery Council (NMC) guidelines, maintaining accurate and up-to-date patient records. Guideline Adherence: Demonstrate a thorough understanding of national and local health guidelines, applying these standards to patient care to ensure high-quality and compliant practice. Additional Responsibilities: Undertake any other delegated duties appropriate to the post, contributing to the efficient running of the practice and ensuring the delivery of high-quality patient care. Managerial: Aware of identification and reporting procedures related to professional standards. To attend staff meetings (when appropriate) Support the Practice clinical governance agenda. Educational: Identify personal development and training needs in conjunction with the manager. Participate in clinical supervision. Support others with their training and development needs. To participate in continuing education and maintain a contemporary level of professional knowledge and skills.
