An enthusiastic and knowledgeable B6 Clinical Nurse Specialist / Practitioner who is committed to delivering and ensuring others deliver excellent, evidence-based respiratory care. You will be part of a senior team, leading and developing nurses, physiotherapists, nursing associates, associate practitioners, physio technicians, and administrative support to deliver care for people with respiratory illness in the place of choice. A special interest and knowledge in COPD would be beneficial.
Main duties of the job
The successful candidates will provide specialist respiratory intervention, as well as supporting the multidisciplinary team in assessment and development of a holistic management plan. You will be expected to perform assessment of respiratory patients with diverse presentations and complex physical and psychological conditions, developing and delivering an individualised physiotherapy treatment programme based on this. You will understand the assessment and management of multiple respiratory conditions and be aware of local and national guidelines for COPD, Bronchiectasis, Interstitial Lung Disease, Asthma, and Oxygen.

The role is predominantly Monday - Friday with the hours being 9-5, although there is a longer-term plan to provide a 7-day service.
The role is available in Frimley Health based in Farnham Community Hospital covering North-East Hampshire and Farnham and Surrey Heath.

Job responsibilities
1. Assessing and reviewing respiratory patients at home as part of the Supportive discharge, admission prevention or acute respiratory virtual ward.
2. Visit patients in their homes, using advanced assessment skills plan, deliver and evaluate patient care during their period of acute illness, and have advanced understanding of the complexity of issues surrounding the patient and their care.
3. Using effective counselling/educational skills which may involve breaking bad news on diagnosis or prognosis of a chronic condition. To contribute to palliative care of respiratory patients as they approach end of life.
4. Assessing and reviewing patients who are on long-term oxygen therapy within their usual place of residence.
5. To provide oxygen assessment to those patients referred to the service within the clinic setting, considering the most appropriate modality should oxygen be required.
6. Assess acutely unwell patients with respiratory illness in particular COPD who have been admitted to the Acute Hospitals (FPH) with the view to supporting discharge. This will involve reviews of their physical needs, their social and psychological circumstances and will require advanced assessment skills in those areas.

Essential
* Registered Nurse - Part 1,2,12.
* First degree in nursing, or demonstrable experience at this level
* Teaching / assessment programme
* Relevant specialist post basic programme in respiratory / experience or relevant community nursing qualification
Desirable
* Diploma/ degree in respiratory care (COPD / Asthma) or willing to complete
Skills
Essential
* Demonstrates up to date knowledge in general nursing
* Knowledge of key professional issues and NMC guidelines and codes, relating to professional practice
* Awareness of community working and structures
* Knowledge of the COPD and asthma outcomes strategy, NICE & BTS guidelines
* Clinical knowledge & skills in respiratory care
* Able to work autonomously and within a team.
* Evidence of continuing professional development
* Previous experience of formal / informal teaching of patients & staff
* Decision making skills.
* Demonstrates skilled written and verbal communication skills.
* Keyboard and word processing skills
* Full driving licence and access to a car for use to travel across sites and perform domiciliary visits.

* Minimum 2 years relevant clinical experience in the same or related specialty OR at least 2 years recent clinical experience in the acute or community setting

* Evidence of ability to work collaboratively.
* Previous experience in audit and evaluation
* Experience in acting as a clinical supervisor and mentor to others.
Desirable
* Previous community nursing experience
* CBG / ABG interpretation
